New codes of insurance titles and new ZUS declarations

On November 1, 2013, the Regulation of the Minister of Labor and Social Policy of September 9, 2013 entered into force, which changes the existing regulation on specifying forms of applications for social insurance and health insurance, personal monthly reports and personal monthly corrective reports, payer's notifications, settlement declarations and corrective settlement declarations, notifications of data on work in special conditions or of a special nature, and other documents (Journal of Laws of 2013, item 1101). In addition, the regulation introduces changes to the models of insurance documents, i.e.

New insurance title codes

This regulation introduces a number of new insurance codes and also modifies those that are currently in force.

In accordance with Annex 18 to the above-mentioned the regulation creates new insurance codes that relate to:

a) persons on parental leave:

  • 12 50 - a person caring for a child who was subject to retirement and disability insurance immediately before taking care of the child as a person running a non-agricultural activity,
  • 12 60 - a person caring for a child who was subject to retirement and disability insurance as the contractor immediately before providing care,
  • 12 70 - a person caring for a child who was subject to retirement and disability insurance immediately before taking care of the child as a person cooperating with a person conducting non-agricultural activity or with the contractor,
  • 12 80 - a person caring for a child who was subject to old-age and disability pension insurance as a clergyman immediately before taking care of the child,
  • 12 90 - a person who takes personal care of a child, subject only to old-age pension insurance,

b) persons receiving a bridging pension or teachers' compensation benefit:

  • 26 00 - a person receiving teacher's compensation benefit, residing in the Republic of Poland,
  • 26 01 - a person receiving teacher compensation benefit residing in a Member State of the European Union other than the Republic of Poland or a Member State of the European Free Trade Association (EFTA),
  • 27 00 - a person receiving a bridging pension residing in the Republic of Poland,
  • 27 01 - a person receiving a bridging pension and residing in a Member State of the European Union other than the Republic of Poland or a Member State of the European Free Trade Association (EFTA),

c) benefits and breaks:

  • 319 - maternity allowance from sickness insurance for the period determined as the period of parental leave,
  • 320 - compensation of maternity benefit from sickness insurance for the period determined as the period of parental leave,
  • 325 - maternity allowance from sickness insurance for the period determined as the period of additional maternity leave or additional leave under the conditions of maternity leave,
  • 326 - compensation of maternity benefit from sickness insurance for the period determined as the period of additional maternity leave or additional leave under the conditions of maternity leave,
  • 327 - maternity allowance from sickness insurance for the period determined as the period of paternity leave,
  • 328 - compensation of maternity benefit from sickness insurance for the period determined as the period of paternity leave,

d) section 1. Basic entity with extension:

  • 19 10 - a person who is subject to voluntary retirement and disability pension insurance during the period of suspension of business activity.

The change will also apply to other codes, affecting their meaning:

  • 311 - maternity allowance from sickness insurance for the period determined as the period of maternity leave or leave on the terms of maternity leave,
  • 315 - equalization of maternity allowance from sickness insurance for the period determined as the period of maternity leave or leave on the terms of maternity leave.

The introduction of the regulation will result in the removal of the following codes: 10 40, 15 00, 16 10, 17 00, 18 10.

Declaration changes

Due to changes in insurance codes, and thus changes to the Act on the social insurance system, modifications to the templates of insurance documents will be introduced. This applies to the following forms:

  • ZUS ZUA - application for insurance / notification of a change in the personal data of the insured person,
  • ZUS ZCNA - reporting data on family members for health insurance purposes,
  • ZUS ZZA - application for health insurance / notification of data change,
  • ZUS ZIUA - notification of a change in the identification data of the insured person,
  • ZUS ZWUA - deregistration from insurance,
  • ZUS RCA - personal monthly report on contributions due and benefits paid,
  • ZUS RZA - personal monthly report of due contributions for health insurance,
  • ZUS DRA - settlement declaration,
  • ZUS ZSWA - reporting and correction of data on work in special conditions or of a special nature.

Both premium payers who have the existing forms of insurance documents and submit them in writing, and those who transfer them using the software, can use the current forms until the end of 2013.

Users of the existing document templates must use such insurance title codes and benefit / break codes which are valid until October 31, 2013.
